My bill for the Sunday i spent in the ER was $1667.

I paid $748.

My insurance paid $1500.

What's wrong with this picture?

Heh.. seriously though, the insurance 'math' is nuts. You have mandatory set ER fees that you are responsible for, then adjustments, then insurance paid, then insurance adjusted, then in about 3-6 months you will get another bill for everything your insurance didn't fork over.
